Registered Nurse

Position Overview

To provide nursing care in accordance with the patient’s plan of care. This includes comprehensive health and psychosocial evaluations, monitoring of patient condition, health promotion and prevention, coordination of services, training activities, and direct nursing care.


Key Responsibilities

  • Coordinate total patient care through comprehensive health and psychosocial evaluations, monitoring, prevention, and training.
  • Continuously evaluate the effectiveness of nursing services provided.
  • Manage patient admission, transfer, re-certification, resumption of care, and discharge paperwork.
  • Present patient records to the Clinical Record Review Committee when necessary.
  • Consult with attending physicians to adjust patient care plans as needed.
  • Coordinate and document patient services, including tallying daily visits.
  • Submit clinical and progress notes as per policy (weekly or within 72 hours).
  • Participate in case conferences and discuss patient-related concerns with supervisors.
  • Coordinate with other health professionals and services to ensure comprehensive care.
  • Participate in discharge planning and staff development meetings.
  • Stay current with drug therapy knowledge and federal/state requirements.
  • May be asked to fill in for other nurses as needed.

Admission Duties

  • Conduct comprehensive patient assessments and obtain medical history.
  • Perform physical and environmental evaluations of the home and patient.
  • Determine the need for additional services such as CHHHA or Home Health Aide.
  • Develop and implement individualized nursing care plans.
  • Educate patients and families about available nursing services.

Nursing Care Duties

  • Provide skilled nursing services as outlined in the care plan and prescribed by the physician.
  • Initiate preventative and rehabilitative procedures.
  • Observe and report patient condition changes and treatment reactions.
  • Educate and supervise patients and caregivers regarding home care needs.

Supervision of CHHHA (Certified Home Health Aide)

  • Supervise and evaluate CHHHAs as per policy.
  • Conduct and document supervisory visits and evaluations.
  • Provide performance feedback to CHHHAs in collaboration with the Aide Supervisor.
  • Offer guidance and supervision to LPNs.
  • Update and revise care plans as needed.

Job Conditions

  • Must possess a valid driver’s license and be willing to drive to patient residences.
  • Ability to access non-wheelchair accessible homes is required.
  • Must be physically capable of performing patient assessments and care.
  • Good verbal and written communication skills in English are essential.

Company Information

Access to confidential patient medical records, personnel files, and financial information is required. Communication with the Director of Nursing is essential regarding these records.


Qualifications

  • Graduate of an accredited School of Nursing
  • Valid Registered Nurse (RN) license in the state of New Jersey
  • Preferred: 1+ years of experience in home health or hospital settings
  • Knowledge of professional nursing principles and home care practices
  • Strong time management and organizational skills
  • Excellent communication and teamwork capabilities
  • Must pass a criminal background check
  • Current CPR certification required
